Why did Health and Welfare decide to contract its Res Hab Program Coordination services to only one agency?

In a letter to CFH Providers, dated 6/10/11, the Idaho Dept. of Health & Welfare’s Medicaid Division explained: “Over the past few years, Idaho Legislation has directed the Department to enter into selective contracts for services. This direction was first provided to the Department in 2006, subsequently in House Bill 701 in 2010, and finally by direction contained in House Bill 260 in 2011. In compliance with this legislative direction, the Department would like to announce that a contract has been awarded to Community Partnerships of Idaho, Inc. for ‘Residential Habilitation Program Coordination for Certified Family Home Providers’.”

What services will Community Partnerships of Idaho (CPI) provide to Certified Family Home (CFH) Providers?

Services which will be provided as part of the Idaho Department of Health & Welfare’s contract with Community Partnerships of Idaho (CPI) include (but are not limited to) the following:

  • New provider CFH Orientation and Training
  • Provider Skills Training related to implementation plans
  • Access to training modules and community resources on our website:  www.cpireshab.com
  • Quarterly phone contact to review implementation plans
  • Face-to-face meetings one time per year
  • Training at a yearly face-to-face meeting related to implementation plans
  • Follow-up phone calls to providers as needed to discuss implementation program plan changes.
  • Completion of required provider status review.

When will I meet the new Program Coordinator? 

Program Coordinators will begin contacting CFH providers mid March.  In March and April we will be hosting local meetings with CFH providers, participants, family members and service coordinators to introduce our Program Coordinators and answer questions about the contract.

What do I do if I have questions about my CFH before the new Program Coordinator contacts me? 

If you have questions about your CFH requirements or an upcoming inspection, you may contact the CFH inspectors in your region. Regional contacts are listed on the following website: www.cfh.dhw.idaho.gov

I heard that Program Coordinators will only be coming to visit my home one (1) time per year.  Is that true?

Yes.  Under the new statewide contract Program Coordinators are required to complete one annual site visit per year.  As Program Coordinators will no longer be attending individual support plan (ISP) meetings, they will work with CFH providers to schedule this meeting before the participants annual ISP to review all of the home programs and to help the CFH prepare for the ISP.  At these annual meetings, Program Coordinators will provide training on formal program implementation plans (PIPs), update participant information, obtain signatures on releases and complete a participant satisfaction survey. 

Will my Targeted Service Coordinator (TSC) be changing?

No. The statewide contract is solely for Program Coordination for Certified Family Homes and does not affect the TSC that is working with the participant(s) in the home.

If I have concerns about any case management or crisis, who should I contact for assistance?

The recommended contact for assistance is the participant’s Targeted Service Coordinator (TSC). The TSC should be contacted:

  • When it is necessary to change a participant’s individual service plan (ISP) or specific goals or actions because of the participant’s medical, psychiatric, social, educational or other service needs.
  • When the participant needs help connecting with activities related to their medical, psychiatric, social, educational or other service needs.
  • When monitoring or follow up service coordination activities are necessary due to a change in the status of the participant, making it necessary to adjust the plan and arrange additional services.
  • When a participant is in crisis and needs to access community resources in order to resolve a crisis.

CFH providers should always contact 911 for a medical emergency or a crisis involving the participant when there is an imminent threat of harm to self or others

How will Community Partnerships of Idaho obtain participant files?

To assist in a smooth transition, CPI has requested a copy of your participant’s current individual support plan (ISP) from his/her Targeted Service Coordinator (TSC).  In addition, we have requested copies from Residential Habilitation Agencies of participant’s current Residential Habilitation Plan, Program Implementation Plans, and Provider Status Review.  If CPI is unable to receive this information, assistance will be needed from the CFH to obtain copies of these documents from their home files.  If this is the case, the Program Coordinator will let the CFH know when they call them in March.

Who will assist me with my billing when the contract starts?

Billing Directly to Molina: You may submit your own billing to Molina.  You can access this information on the state’s CFH website: www.cfh.dhw.idaho.gov.  Molina representatives may also be available at our upcoming local meetings to offer hands on training.
